Client understands Allie Andrews (herein referred to as “Consultant”) and OmBody Health, is not an employee, doctor, manager, therapist, registered dietician, or psychotherapist. Client understands that Consultant has not promised, shall not be obligated to and will not; (1) provide one-on-one coaching or support; (2) prescribe anything specific to the Clients conditions (all practices will be generalized recommendations); (3) act as a therapist providing psychoanalysis, psychological counseling or behavioral therapy; (4) act as a nutritionist or dietician. Client understands that a relationship does not exist between the parties after the conclusion of this program. If the Parties wish to continue their relationship, they shall execute a separate agreement.
Client understands that the services being provided by or on behalf of OmBody Health, LLC (“OmBody”) and Allie Andrews, includes, among other things, physical movements such as yoga, meditation, physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension, and dietary and lifestyle changes (the “Practices”)
Participating in the Practices may include, but is not limited to, participation in meditation techniques, yogic breathing exercises, and performing various yoga postures. Yoga postures, or asanas, are designed to exercise every part of the body – stretching and toning the muscles and joints, the spine and the entire skeletal system. They also work on the internal organs, glands and nerves. Yoga incorporates sustained stretching to strengthen muscles and increase flexibility. Yoga is an individual experience.
As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. Client acknowledges that they will participate in these Practices at their own pace. If they experience any pain or discomfort, they will listen to their body.
Client understands that OmBody is not acting in the capacity of a doctor, licensed dietician-nutritionist, psychologist or other licensed or registered professional, and that any advice given by or on behalf of OmBody is not meant to take the place of advice by these professionals. If Client is under the care of a health care professional or currently use prescription medications, they understand that they should discuss any dietary changes or potential dietary supplements use with my doctor, and should not discontinue any prescription medications without first consulting their doctor.
Client has chosen to work with Allie Andrews and OmBody and they understand and agree that the information received should not be seen as medical or nursing advice and is not meant to take the place of seeing licensed health professionals.
Client understands and agree that OmBody does not and will NOT guaranty any particular results and that the success of the Services depends on their goals and many factors outside the control of OmBody.
Client acknowledges and agree that I take full responsibility for their life and well-being, as well as the lives and well-being of their family and children (where applicable), and all decisions made during and after participation in the Practices.
Client hereby expressly assumes the risks of the Practices, including the risks of trying new foods or supplements, and the risks inherent in making lifestyle changes.
